JOB SUMMARY
The Behavioral Health (BH) Clinical Care Manager utilizes advanced clinical judgment and critical thinking skills to facilitate appropriate physical and behavioral healthcare and social services for members through assessment and member-centered care planning, direct provider coordination/collaboration, and coordination of psychosocial wraparound services to promote effective utilization of available resources, optimal member functioning, and cost-effective outcomes.
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Assessment of members through the use of clinical tools and review of member specific health information/data, conducts comprehensive assessments of referred members needs/eligibility and, in collaboration with the members care team, determines an approach to resolving member issues and/or meeting needs by evaluating the members benefit plan and available internal and external programs/services and resources.
- Applies clinical judgment to the incorporation of strategies designed to reduce risk factors and barriers and address complex clinical indicators which impact care planning and resolution of consumer/member issues.
- Using advanced clinical skills, performs crisis intervention with members experiencing behavioral health or medical crisis and refers them to the appropriate clinical and service providers for thorough assessment and treatment, as clinically indicated.
- Monitoring, Evaluation and Documentation of Care while performing the enhancement of Medical Appropriateness and Quality of Care.
- Conduct assessment of risk, mental status and medical stability of clients that are in need of Crisis Intervention and follow up with Bayou Health carrier to obtain crisis units
- Conduct 1915 assessment of all new clients and existing if necessary, receive/review new referrals and obtain authorization of units before assigning to a Care Coordinator
- Gather documentation and complete quarterly outcomes and data for LANO reports
- Facilitate clinical staff meetings
- Approve time sheets and mileage reimbursement for team members
- Provides training and implementation of manager care or organizational requirements
- Monitors required performance standards
- Interface as the liaison between manager care organization and leadership of Volunteers of America Southeast Louisiana (VOASELA)
SUPERVISORY DUTIES
Supervision of PSH Team members assigned Supervision of Interns and Licensure
KEY CONTACTS
- Serves as primary representative of the Agency with respect to third party vendors providing training-related products and services
- Utilizes the Relias Learning Management System to maintain compliance with all required training courses
This position requires the incumbent to have a Master's Degree in social work or counseling.
Louisiana licensure as a Licensed Mental Health Professional (e.g. LCSW, LPC, LMFT, LAC, APN, PhD, or PsyD.) required. Level of Care Utilization System (LOCUS)/CALOCUS certification, case management, discharge planning experience, managed care, and crisis intervention skills are strongly preferred. Candidates without current certification will be required to obtain certification post hire. 8 years management and/or post graduate clinical practice experience (e.g., hospital setting, alternative care setting, home health or ambulatory care) required. This position requires the ability to communicate clearly in person and/or via telephone communication, and in writing. Requires the ability to provide services to the target population, organizational skills, and the ability to prioritize multiple tasks concurrently. SPECIAL REQUIREMENTS
Travel to other locations as appropriate in the performance of duties and responsibilities. Must have:
- Current driver’s license issued by state of residence
- Current Vehicle Inspection decal
- Current vehicle insurance
- Current vehicle registration in state of residence
- Satisfactory Motor Vehicle Report
- Must be able to pass a criminal history investigation
- Be able to obtain Cardio Pulmonary Resuscitation, First Aid certifications
