Position: Case Manager
Reports to: Housing Support Director
Starting Salary: $55,000
FLSA Status: Exempt
Function:
Under general supervision, the Case Manager performs a wide range of tasks in the community to assist individuals in regaining control over their own lives while promoting the recovery process. Utilizing psychiatric rehabilitation interventions provides skills training, education, and case management to individuals and their families. Monitor individuals' progress toward treatment goals. Extensive documentation is required; accurately charts services and individual progress. Must be able to work well with community agencies and function as part of a team.
Qualifications:
Bachelor’s degree in social work, psychology, mental health, nursing, education, sociology, criminal justice, counseling, rehabilitation counseling, guidance counseling, or an equivalent field AND one year of full-time experience in social services delivery. The ability to work in a stressed filled environment to meet deadlines and deal with routine and crisis is required. Knowledge of mental health is required. Must possess a valid GA driver’s license and continually meet the requirements for VOASE’s vehicle insurance company.
Principal Activities:
- Provides direct services to individuals and their families and meets the established productivity goals, maintains timely authorizations, and ensures timely documentation that meets CARF/DCA/ASO requirements.
- Participates with other treatment team members in identifying the individual's strengths and weaknesses and developing a care/service plan.
- Schedules follow-up appointments promptly to ensure services are provided.
- Ensures all Community Service Board appointments are kept and if not informs the supervisor.
- Refers individuals to a PCP if they do not have one and ensures ongoing communication with the PCP.
- Coordinates and/or makes appropriate referrals for other support services as needed.
- Complete progress notes and prepares and disseminates pertinent reports for/to supervisors, colleagues, and other appropriate individuals promptly.
- Provides exceptional customer service by engaging and building rapport with individuals and all stakeholders.
- Attend and actively participate in all meetings/staffing/training as scheduled to ensure knowledge of administrative changes/directives and provide/receive clinical information.
- Quality individual satisfaction. Optimize continuous improvement in quality-of-care individual safety and the individual experience.
