Essential Job Functions:
- Assure that social service/discharge planning activities are integrated and coordinated within the facility team.
- Develop specific, measurable, realistic plans and objectives, which enhance social service daily.
- Perform psychosocial assessment and reassessment on all residents. Identify and address the psychosocial needs of residents and their families.
- Demonstrate knowledge of clinical social work and discharge planning. Interpret and promote residents’ rights.
- Interact professionally with resident and family and involve resident and family in the formation of the plan of care.
- Provide individual and group help for residents’ and families at times of adjustment, crisis, or need.
- Coordinate and direct Social Service/Discharge Planning Department to ensure resident needs are met.
- Provide appropriate resources and support to Social Service/Discharge Planning Staff/Clinical Staff.
- Be flexible, organized, and function under stressful situations.
- Maintain a good rapport and working relationship with medical staff, departments, and employees.
- Manage and operate equipment safely and correctly.
- Treat patients and their families with respect and dignity.
- Support and maintain a culture of safety and quality.
