The Social Services Coordinator (SSC) is responsible for managing and delivering comprehensive social services to the residents of Fredrick Samuels Apartments, a NYCHA Housing Development. The SSC ensures that services are tailored to meet the diverse needs of seniors, families, youth, and teens, facilitating access to essential resources, programs, and community partnerships. Acting as an advocate for residents, the SSC connects them with external services such as healthcare, vocational training, financial assistance, and counseling, while collaborating closely with the Senior Manager of Tenant Relations and Social Service to ensure the effective integration of support services. This role is integral to fostering a supportive community environment and improving residents' overall quality of life.

Essential Duties And Responsibilities
The Social Services Coordinator will conduct all business in accordance with company policies and procedures, and city, state and federal laws (OSHA, ADA, Fair Housing and NYCHA). Essential functions include, but are not limited to the following:
- Resident Support and Advocacy: Conduct assessments to identify resident needs, linking them with community services such as day programs, vocational services, mental health counseling, and community activities. Provide individual counseling and support services, addressing residents' social, emotional, mental, and financial challenges. Advocate for residents, ensuring timely and appropriate referrals to local service providers, government agencies, and community organizations. Develop and implement programs and workshops aimed at enhancing residents' life skills, including financial literacy, job readiness, and health education.
- Case Management: Create quarterly goal plans, conduct functional assessments, and review residents' progress through periodic meetings, adjusting support plans, as necessary. Maintain accurate and up-to-date documentation for each resident, including progress notes, completion data, and compliance.
- Crisis Intervention: Respond to resident crises, including eviction prevention, domestic violence, or health emergencies, by coordinating with external agencies to provide immediate assistance. Develop individualized safety plans and refer residents to emergency services when necessary.
- Collaboration and Coordination: Collaborate with external partners and community organizations to provide residents with comprehensive support. Participate in staff supervision meetings, tenant meetings, and NYCHA-related events to stay informed and address resident needs. Work closely with property management team to ensure a cohesive approach to tenant relations and social services.
- Reporting and Compliance: Prepare and submit reports on service utilization, resident outcomes, and program effectiveness for internal and external stakeholders.
- Other duties as assigned
- High school diploma or equivalent; Bachelor's degree in social work, Human Services, Psychology, or a related field.
- Licensed social worker (LSW) or equivalent certification preferred but not required.
- At least 2 - 3 years’ experience in social services, case management, or a related field.
- Experience working with low-income or vulnerable populations is highly desirable.
- Strong case management, advocacy, and crisis intervention skills.
- Excellent communication (verbal and written) and interpersonal skills for effective collaboration with residents, staff, and external partners.
- Ability to develop and maintain effective partnerships with service providers, government agencies, and community organizations.
- Strong proficiency in Microsoft Office Suite and case management software.
- Knowledge of NYC community resources, public benefits programs, and housing assistance.
- Ability to handle sensitive information with discretion and maintain confidentiality.
- Bilingual (English/Spanish) is preferred.
- Highly organized and ability to prioritize matters and manage time effectively.

- This is a fast-paced, in-person office role within an affordable housing community. The Social Service Coordinator works on-site, directly interacting with residents and community partners. Some travel to offsite meetings and community events may be required to support residents and build partnerships.
