Intake Specialist

Job Summary:

The Intake Specialist will travel within the assigned county and neighboring areas to conduct in-home intake visits for applicants. This role facilitates the eligibility determination process, guiding individuals from initial identification through enrollment or termination.

Key Responsibilities:

• Conduct in-home intake visits and support applicants through the eligibility process.
• Explain program details, ensuring applicants understand their rights and responsibilities.
• Assist with application submission, including data entry via the Compass Application.
• Travel as needed to meet business requirements.

Qualifications:

• Experience in case management, social work, or a related field preferred.
• Strong communication and organizational skills.
• Ability to travel within the assigned region.

Job Type: Contract

Pay: $18.18 - $20.50 per hour

Expected hours: 40 per week

Schedule:

• 8 hour shift
