What are the responsibilities and job description for the Outreach Education Worker/Navigator position at Waianae Coast Comprehensive Health Center?
POSITION SUMMARY:
This position is responsible for conducting outreach and education on facilitating appropriate use of the Center’s resources, providing enrollment assistance for eligible individuals interested in applying for Public Benefits Entitlements. This position also is responsible for connecting high-risk Medicare and Medicaid beneficiaries and facilitate access to community service providers (CSPs) that can meet their health related social needs. This position actively participates in special activities of the Department. It is expected to work within the program directives of the funding source, within the philosophical framework set by the Board of Directors, and within the existing network of community services providers and natural helpers.
EDUCATION/EXPERIENCE:
1. High School Diploma or equivalent required
2. Community Health Worker Certification or college degree preferred
3. Three years of experience in a health care setting preferred
4. Experience and competency in Public Benefit Applications and community outreach work preferred
5. First Aid/CPR Certified
6. A current Hawaii State Driver’s License, daily access to an automobile with current registration, safety check and the minimum liability insurance as required by the “Hawaii No-Fault Law” required
