TRULY Board Eligible (within 5 years of residency) or Board Certified in Family Medicine (preferred) or Internal Medicine. IN license (or willing to obtain) Active DEA (or willing to obtain) BLS and ACLS (or willing to obtain) COVID Vaccination Provider will be working with all ages - must have experience working with pediatrics-geriatrics EPIC EMR knowledge
