Case Manager III
Case manages assigned group of 15-20 clients in the single women’s program. Develop individualized strategies to improve their lives and support successful transition from homelessness to independent living. Provide client-centered, culturally sensitive treatment approach that promotes recovery, symptom reduction, increased coping skills and the achievement of the highest level of social functioning. Collaborate effectively with an integrated Program team to establish and carry out plans for residents. Opportunities to share faith within a supportive workplace.
- Case Manager participates in:
- Weekly case consult meeting and women’s team meeting
- Coordination of care with outside service providers
- Provision of weekly case management services
- Holds 1-2 groups for residents per week
- Supports residents in learning daily life skills
- Enforces program requirements and actively engages residents in following goal plan
- Education and experience:
- Bachelor's degree required and experience in recovery or treatment fields highly preferred.
