Responsibilities

Performs outreach to patients
Schedules appointments
Coordinates with clinics to ensure appropriate appointments
Tracks details on excel spread sheet
Drill down of care gaps

Physical Demands :

While performing the duties of this job, the employee is regularly required to talk or hear. The employee frequently is required to stand; walk; use hands to finger, handle or feel; and reach with hands and arms. The employee may occasionally lift up to 10 pounds.

Qualifications and Education Requirements

High School Diploma and 1-2 years of experience

Experience with Microsoft Office, Epic System, running reports and data mining

Experience in Clinical Operations

Must have strong organizational skills to effectively prioritize, coordinate, perform and complete multiple duties and assignments concurrently and in a timely manner.

Knowledge of and proficiency in English composition, spelling, grammar, punctuation and basic math principles

Strong presentation, interpersonal and communication skills
