JOB SUMMARY

Under the general supervision of a higher-classified employee, the responsibility of the hospital/court liaison is to be the key connection between Community Mental Health, community hospitals and the court system.

EXAMPLES OF DUTIES

Essential Duties:



Position will have a primary focus of providing liaison services between community hospitals the court system and CMH.

Conducts utilization management functions on behalf of CMH for General Fund and Medicaid sponsored patients.

Plays key role in coordinating discharges for consumers of CMH as well as community members not engaged with CMH.

Coordination of discharges to community based substance abuse agencies with clients who are co-disordered or who have a primary diagnosis of substance abuse.

Attends hospital treatment team meetings for General Fund and Medicaid sponsored patients while hospitalized.

Attends deferral conferences with legal and hospital personnel, makes recommendations and prepares ATPs for the Washtenaw County probate court.

  • Promotes interagency cooperation and represents the agency in the community with other County departments, outside agencies, with families of customers, and other interested parties in such a way that an understanding of customers' needs are fostered and that fosters the mission and goals of the agency.

Important Duties:



Enters data into a personal or mainframe computer system. Prepares information, correspondence and other written materials using word processing software.

Uses standard office equipment in the course of the work.

May drive a County or personal vehicle in the performance of the work.

EMPLOYMENT QUALIFICATIONS

Knowledge of:



Crisis residential services.

Experience with differential diagnosis between SPMI, SA and medical disorders.

Principles and practices, applicable laws and regulations covering service delivery in the discipline to which assigned.

Knowledge of Michigan Mental Health Code and other applicable laws, policies and regulatory standards.

Knowledge of court processes and required documentation.

Department policies, procedures, rules and regulations.

Techniques of dealing effectively with individuals of varying socio-economic backgrounds.

Community resources available to customer families and individuals.

  • General knowledge of common office computer software programs as well as applications which are specific to the assigned unit/department.

Skill in:



Interpreting, applying and explaining complex laws, policies and regulations.

Analyzing complex personal and procedural problems, evaluating alternatives, and making sound recommendations.

Using sound independent judgment within established policy and procedural guidelines.

Maintaining accurate files and records.

Preparing clear, concise and effective written materials.

  • Establishing and maintaining effective working relationships with those contacted within the course of work.

Ability to:



Represent the agency to partnering agencies, the provider network, the community, consumers and other interested persons in a way that fosters a positive working relationship.

Work with the mental health system and programs.

Coordinate and work with community based housing resources.

  • Mobilize community housing resources.

PHYSICAL DEMANDS

  • Duties require sufficient mobility to work in a normal office setting and use standard office equipment including computer, vision to read printed materials and a computer monitor, and hearing and speech sufficient to communicate in person and over the telephone.

EDUCATION and LICENSURE

Minimum requirements are that applicants have a valid Michigan Driver's License and meet any requirements by funders for transportation of consumers, a Bachelor of Science in Nursing or Master's degree in Social Work or Psychology plus one of the following:

  • License Master's in Social Work (LMSW) State of Michigan certification

OR



  • Limited Licensed Master's in Clinical or Behavioral Psychology (LLP) State of Michigan certification

OR



  • Registered Nurse in the State of Michigan

EXPERIENCE

A minimum of five (5) years performing professional level human services delivery which includes the following:

Working with community based crisis services.

Coordinating and planning discharges.

  • Working with attending physicians and hospital staff.
