What are the responsibilities and job description for the Pharmacist Clinical / Staff position at Waterbury Hospital?
Under the direction of the Clinical Pharmacy Manager, provides advanced pharmaceutical care by assisting in the development and coordination of clinical pharmacy services and educational programs for the Department. Responsible for providing tools and educational opportunities to enhance services provided by all staff members. Participates on intra- and interdepartmental committees as assigned. Serves as preceptor to pharmacy students and residents as assigned.
Participates in the development of institution-wide drug use policies. Perform a variety of duties involving the receiving, storage, compounding and dispensing of medications and supplies. Responsible for the assessment of patient drug therapy, review of drug interactions, incompatibilities, drug induced adverse effects, and appropriate drug doses. Documents the care provided to patients. Participates in supervision of technical staff. Reports to the Clinical Pharmacy Manager.
REQUIREMENTS :
Advanced degree required (PharmD). Must be a graduate of an accredited school of Pharmacy. Must have current pharmacist license in State of Connecticut. Three to five years in-patient pharmacy experience required or completed PGY1 residency. In addition to a PharmD and residency / fellowship, board certification in a pharmacy specialty within the first 3 years of employment is required. Ability to verbally communicate effectively with multiple levels of personnel / interdisciplinary teams, with tact and confidentiality. Must be proficient in all aspects of compounding, preparing and dispensing drugs. Must have working knowledge of federal and state laws as they pertain to drug procurement, storage, dispensing, etc. Must demonstrate ability to maintain a collaborative practice relationship with physicians and other health care professionals.
