What are the responsibilities and job description for the Client Services Coordinator position at Wausaukee Enterprises, Inc.?
- Establish, maintain, and evaluate the adult day service programming of Wausaukee Enterprises, Inc.
- Responsible for case management for all clients
- Serve as a point of contact for all clients, families, guardians and others
- Make recommendations for further services to clients, families, and guardians
- Prepare required progress reports, evaluation reports, and required inter-agency reports
- Provide in-service training to the staff in relation to his/her program area responsibilities
- Work with clients to create and work towards goals and objectives
- Consult with appropriate staff regarding past, present, and future goals and how they are related to specific programming
- Assist disabled adults in adult day services programs
- Supervise and assist with meaningful adult day services programming
- Provide meaningful interaction and maintain a positive attitude
- Support program members to be as independent as possible
- Support program members to achieve their goals
- Implement and carry out member specific support plans
- Perform any other duties related to the position within the policies, goals, and objectives of the organization as assigned
- Complete staff training and development activities as assigned
- Maintain confidentiality
