Position: CSP Case Manager
Location: Waushara County, WI
Salary: $24.57 - $26.07/hour
Waushara County is seeking a compassionate and skilled CSP Case Manager to support adults living with severe and persistent mental illness. Key Responsibilities:
- Develop, implement, and monitor treatment plans for clients, including documentation and regular progress notes.
- Provide case management to individuals diagnosed with severe and/or persistent mental illness.
- Coordinate care with other service providers and assist clients with referrals to necessary resources.
- Support clients with financial management, housing, and other aspects of daily living.
- Provide critical intervention during emergencies, including hospitalization or crisis response.
- Attend staffings and supervision sessions, maintaining open communication with your supervisor.
Qualifications:
- Bachelor’s degree in a behavioral science or related field (preferred).
- 1,000 hours of post-degree clinical experience working with individuals with severe and persistent mental illness.
- Strong knowledge of psychotropic medications and their effects.
- Ability to work independently and as part of a team, with a client-centered approach.
- Valid driver’s license and adequate car insurance for client transportation.
Working Conditions:
- Flexibility with occasional evening and weekend hours required.
- Occasional travel to client locations, and the ability to work with clients in crisis or distress.
- Ability to perform duties independently while following policy and procedures.
