Job Summary:
The RNCM's overall responsibility is for the management, oversight, coordination, and provision of care for hospice patients in the Fairfield County, CT area. The RNCM is an integral member of the Hospice Interdisciplinary Group (IDG). The role requires hands-on, clinical, symptom-focused nursing skills and direct patient care in patient's home environment; interdisciplinary collaboration focused on relieving total symptom burden; and on the provision of goal concordant care for patients approaching end of life and their loved ones. This is accomplished through RNCM’s continuous assessment of each patient and family’s needs, coordination with Hospice IDG, ongoing and open communication with patients, family members, physicians, and Hospice IDG, and the provision of relevant patient/family information and education throughout the trajectory. The Hospice PCN reports to the Hospice Program Clinical Supervisor and Hospice Program Director.
Rates based on experience and visit type.
Qualifications:
1. A current license to practice as a RN in CT.
2. Graduation from a State licensed and Accredited School of Nursing.
3. BSN degree preferred.
4. Must have a minimum of one-year either home health, hospice, and/or inpatient acute care experience.
5. Maintain CPR certification.
6. Strong organizational and time management skills required.
7. Computer proficiency required.
8. Ability to work both independently and also as part of a team.
9. Strong verbal and written communication skills.
Responsibilities:
· Initial and ongoing assessment of the impact of the terminal diagnosis on the patient's physical, functional, psychosocial and environmental needs and activities of daily living.
· Establishes and implements an individualized plan of care. In collaboration with patient/family, physician and Hospice IDG.
· Responsible for ongoing assessment and management of patient reported pain and symptom concerns.
· Provides specialized nursing skills related to palliative and end-of-life care.
· Educates the patient/family regarding the disease process and end-of-life.
· Timely communication of clinical and care plan changes to physician and Hospice IDG.
· Determines scope and frequency of services needed based on acuity and patient/family needs.
· On-going evaluation of patient/family response to care.
· Performs comprehensive skilled assessments of patient needs, appropriateness for service, at appropriate time points.
· Oversight of safe medication administration including patient/family teaching/education and ongoing symptom, side effect, and risk assessments.
· Facilitates case management of patient care with hospice IDG that reflects patients changing needs, incorporating all essential members of patient’s family and other disciplines.
· Completes clinical documentation in the patient record, as required by CMS, State and Agency guidelines.
· On-going assessment and referral to appropriate community services as necessary.
· Attends and participates in weekly Hospice IDG meetings, other team meetings, and Agency in-services as required.
· Provides for the delivery of quality skilled nursing care and treatments, in a compassionate patient-centered manner.
· Establishes a patient centered plan of care that assures patient safety 24/7 with particular attention to anticipating planning for patient needs.
· Demonstrates full competency with Agency clinical software and maintains patient care record via computer in an organized and timely manner. Communicates information using current process and technology available to the Agency.
· Adheres to standards of regulatory, legal, professional, and ethical practice.
· Shares weekday and weekend on-call and night responsibilities as needed.

· Abide by related agency policies and appropriate departmental procedures.
