General Responsibilities:
Case Manager will provide case management services and counseling to clients in a juvenile residential treatment center setting. They will be responsible for documenting all actions according to State and Federal requirements.
DUTIES:
- Conduct intake evaluations and interviews, prepare and maintain individual case records including but not limited to individual program plans, histories, progress notes, discharge summaries, etc.
- Counsel residents on an individual basis, conduct family counseling sessions and conduct group sessions according to program outlines and guidelines.
- Observe client behaviors in order to make recommendations and carry out treatment needs.
- Prepare and submit pass requests for residents on the caseload.
- Other duties as assigned and needed.
QUALIFICATIONS AND EXPERIENCE:
Case Manager will have a Bachelor's degree or above in social work, psychology, or a related field. Experience is preferred, but not required.
NO FELONY CONVICTIONS. Must pass a drug screen and background check and maintain eligibility for continued employment.
Job Type: Full-time
Pay: $45,000.00 per year
