Job Description
We at Wayne Memorial Community Health Centers are seeking a Behavioral Health Care Manager to join our team and support the implementation of behavioral health integration services.
As a Behavioral Health Care Manager, you will be accountable for all Care Management activities for assigned high and moderate members with behavioral health conditions identified and assessed as requiring intensive interventions.
The successful candidate for this role will participate as a core member of the collaborating care team to enhance the overall physical and mental health of patients by providing comprehensive, individualized care, coordinating treatment plans and connecting patient/caregivers to essential resources. Amongst other responsibilities, you will Identify and prioritize the patients’ needs and preferences, develop quantifiable goals and desired outcomes and promote the patient’s ability to self -manage to the greatest extent possible

Job Requirements
- Requires Master’s in Social Work, counseling or a related behavioral health field, or a graduate of an accredited nursing program with current licensure in nursing, or at minimum a Bachelor’s in Social Work or related field,
- Minimum of 2 years of experience with facility-based and/or outpatient psychiatric and substance abuse or substance abuse disorder treatment; or any combination of education and experience which would provide an equivalent background.
Required Skills/Abilities:
- Demonstrates interpersonal skills to interact in an effective manner with practitioners, the interdisciplinary health care team, community agencies, patients, and families with diverse opinions, values, and religious and cultural ideals.
- Demonstrates leadership qualities including time management, verbal and written communication skills, listening skills, problem solving, critical thinking, analysis skills and decision-making, priority setting, work delegation, and work organization.
- Demonstrates ability to develop positive, longitudinal relationships and set appropriate boundaries with patients/families.
- Knowledge of chronic conditions, psychiatric/medical terminology, evidence-based guidelines, prevention, wellness, health risk assessment, and patient education and ability to manage complex clinical issues utilizing assessment skills and protocols
- Knowledge of DSM-5 or most current diagnostic edition
- Possesses intermediate computer skills (operating systems, software, and applications) and demonstrates a willingness to learn more advanced skills
- Ability to work independently, while collaborating with other team members. Serves as a clinical resource person to staff and patients/caregivers.
- Willingness to establish effective working relationships with internal and external customers.
- Strong organizational skills and the ability to prioritize and follow through on multiple projects in a timely manner.
