Job Description
Candidate will coordinate with the interdisciplinary team, patient/family on complex aspects of the patients' care, and serves as the focal communication point for patients and staff. The Case Manager works closely with the attending physician(s), social services, nurses, and other members of the team throughout the continuum of care for identified complex inpatients and outpatients. The Case Manager is a clinician who in conjunction with the interdisciplinary team is accountable for performing utilization review activities, assessing complex discharge plans, effective and efficient utilization of resources, meeting established professional and regulatory/third party payer standards, and who collaborates with the healthcare team and the patient/family to accomplish agreeable outcomes.
Job Requirements
Graduate from an accredited school of nursing (BSN preferred); Current licensure in the State of PA; Minimum of two years of clinical experience with strong clinical skills; Solid computer skills; Demonstrated leadership skills; Knowledge of Core Measure and Meaningful Use requirements as they pertain to patient care preferred; Strong understanding of Meditech EMR functions preferred; Supervisory experience preferred; Certification in Case Management (or equivalent) or within one year.

Frequently: (Exists 25-75% of the time) Sitting with and without back support; reaching (overhead, forward, lateral, low); stooping (bend at waist); crouching (bend at knees); balance; lifting 10-55 lbs.; carrying 0-5 and 10-25 lbs.; pushing and pulling 15 - >100 lbs. (help is available).
Occasionally: (Exists 5- 25% of the time) Lifting 55-75 lbs.; exposure to radiation, chemical hazards, infect hazards, slippery surfaces.
Rarely: (Exists under 5% of the time) Kneeling; climbing; lifting 75-100 lbs.; carrying 25 - >100 lbs.; pushing and pulling 0-15 lbs.; operating machinery.
