The Case Manager for Individual Support Services supports each assigned individual to live independently in a safe and healthy environment.
LOCATION: Haverhill, MA with frequent travel in the region
HOURS: 27 Hours/week to include days, afternoons, evenings and some weekends, based upon Individuals' needs
KEY ACTIVITIES:
- Case management tasks including medical coordination and appointments, obtaining medications, assistance with employment, legal, and financial concerns
- Ongoing behavioral assessment to determine ability of the individual to manage life challenges and support needed to do so.
- Assist assigned individuals to achieve their ISP goals
- Teach assigned individuals needed daily living skills, including: household management and cleaning, personal hygiene, healthy life style diet exercise, stress, budgeting, following medical advice
- Assist assigned individuals to develop and maintain healthy relationships
- Record data, and write weekly and quarterly progress notes, maintain records in accordance with DDS regulations
OTHER DUTIES:
- Assist in accessing Personal Care Attendant (PCA), VNA services, therapy, transportation, medical care as well as recreation, housing, adult education or other needed community services
- Participate in the development and implementation of the Individual Support Plan
- Attend regularly scheduled staff meetings , training and meetings with supervisors
- Assist individual with self-advocacy
- Frequent apartment checks for cleanliness and safety
- Support parents with children to practice effective parenting skills.
- Other duties as assigned by the ISS Case Manager Supervisor.
Qualifications
REQUIREMENTS:
- High School Diploma and one year experience working with the developmentally disabled population.
- Must have a valid drivers license and a minimum of one year of driving experience.
- Reading, writing and math skills required.
- CPR/First Aid Certifications are required. Depending on the need of the individuals we serve, the Agency may require you to pass and maintain MAP certification. Knowledge of community resources is helpful and comfort with home visits in many different neighborhoods is essential.
- Computer literacy and the ability to document electronically
