Overview
We are seeking a dedicated and compassionate Case Manager to join our team. The ideal candidate will play a pivotal role in supporting individuals with developmental disabilities and behavioral health needs. This position involves program development, relationship management, and effective program management to ensure the delivery of high-quality services. The Case Manager will work collaboratively with clients, families, and multidisciplinary teams to create and implement personalized care plans that promote independence and enhance quality of life.
Responsibilities
- Develop, implement, and monitor individualized care plans for clients with developmental disabilities and behavioral health challenges.
- Build and maintain strong relationships with clients, families, and community resources to facilitate access to necessary services.
- Manage program operations, ensuring compliance with relevant regulations and standards.
- Conduct assessments to identify client needs and develop appropriate interventions.
- Provide supervision and support to staff involved in client care.
- Collaborate with healthcare providers, social workers, and other professionals to coordinate comprehensive support services.
- Maintain accurate documentation of client progress, service delivery, and program outcomes.
- Participate in budgeting processes to ensure efficient allocation of resources for programs.
- Advocate for clients’ rights and needs within the community.
Requirements
- Bachelor's degree in Social Work or a related field; relevant experience may substitute for education.
- Experience in program development and management within the behavioral health or developmental disabilities sector is preferred.
- Strong relationship management skills with the ability to engage effectively with diverse populations.
- Knowledge of social work principles, practices, and ethics.
- Excellent administrative skills including documentation, reporting, and budgeting capabilities.
- Proven ability to supervise staff effectively while fostering a collaborative team environment.
- Strong organizational skills with the ability to manage multiple priorities simultaneously.
