Overview
We are seeking a dedicated and organized Care Coordinator to join our dynamic healthcare team. The Home Health Care Coordinator is responsible for the effective scheduling and coordination of home health aides to ensure timely and appropriate care is provided to clients. This role involves direct communication with aides, clients, and office staff, as well as handling administrative duties related to scheduling, staffing, and client care.
Responsibilities
- Coordinate daily and weekly schedules for home health aides based on client care needs and availability.
- Assign aides to clients, considering qualifications, client preferences, and geographic location.
- Ensure coverage for open shifts, absences, and emergencies by adjusting schedules and finding replacement.
- Serve as the primary point of contact for clients regarding care schedules, concerns, and service inquiries.
- Conduct client intakes to assess care needs and schedule appropriate services.
- Maintain Administrative and Office Support.
- Assist in payroll processing by verifying and submitting aide work hours.
- Collaborate with the billing department to ensure accurate client invoicing and aide payment.
- Manage communications (phone calls, emails, etc.) from clients and staff in a timely and professional manner.
- Track client care schedules to ensure services are delivered on time and in accordance with care plans.
Requirements
Education: High school diploma or equivalent required; associate’s or bachelor’s degree in healthcare administration, business, or related field preferred.
Experience: At least 2 years of experience in scheduling, coordination, or administrative roles, ideally within a healthcare or home health setting.
Skills:
o Strong organizational and multitasking skills.
o Excellent communication and interpersonal abilities.
o Bilingual (Spanish)
o Proficiency in scheduling software and office systems (e.g., MS Office Suite, electronic health records).
Preferred Qualifications:
· Prior experience in home health, assisted living, or healthcare administration.
· Familiarity with Medicaid, Medicare, and private insurance billing practices.
· Knowledge of medical terminology and home care procedures.
