We are looking for a dedicated Registered Nurse (RN) to join our Group Adult Foster Care (GAFC) program. The ideal candidate will be passionate about providing high-quality care to elders and adults with disabilities.
Qualifications
- Must be fully licensed by the Massachusetts Board of Registration in Nursing.
- Must have at least two years of recent experience in direct care for elders or adults with disabilities.
- Strong clinical assessment and care planning skills.
- Ability to train, evaluate, and supervise direct care aides.
The GAFC RN will be responsible for:
- Completing and coordinating all applicable clinical assessments and evaluations.
- Developing and continuously reviewing each member’s GAFC plan of care.
- Selecting, training, evaluating, and supervising GAFC direct care aides in collaboration with the care manager.
- Reviewing the PCP Summary Form.
- Monitoring the health status and treatment plans of all members to ensure proper care delivery.
- Reporting any changes in health status to the member’s PCP.
- Reviewing GAFC direct care aide logs every 30 days.
- Conducting on-site visits with each member in compliance with 130 CMR 408.505(B)(6).
- Completing nursing progress notes for each visit or as needed based on the member’s condition.
- Submitting semi-annual health-status reports to the member’s PCP.
- Planning and implementing discharges and transitions from the GAFC program.
- Conducting orientation for each GAFC direct care aide before they begin providing personal care.
- Providing ongoing training to GAFC direct care aides on health and aging.
Salary : $40 - $45
