About the Role
As a Primary Care Physician with WelbeHealth, you will be responsible for providing high-quality primary care to patients in our PACE program. This includes conducting regular home visits to ensure continuity of care, collaborating with other healthcare professionals to develop and implement patient care plans, and participating in quality improvement initiatives to enhance patient outcomes.
Requirements
* Board certification in Internal Medicine, Family Medicine, Emergency Medicine, or Geriatrics
* Eligibility for enrollment to provide services to Medicare and Medi-Cal participants
* Strong communication and interpersonal skills
* Ability to work collaboratively as part of a team
