Job Description
This full-time position is located in our El Cajon Clinic and involves providing primary medical management in collaboration with other St. Paul's PACE physicians and providers. The ideal candidate will have a strong background in geriatric care and excellent communication skills.
Responsibilities
- Perform initial and semi-annual assessments and develop plans of care as needed
- Evaluate participant physical complaints and provide appropriate treatment
- Provide participants and caregiver teaching and education
- Round in St. Paul's PACE contracted nursing homes, Assisted Living facilities, Board and Care facilities and participants' homes
Requirements
- Certification as a MD with the appropriate perspective authority
- Minimum of 1 year of experience working with the frail or elderly
- Current driver's license and proof of insurance
- Medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact
