American Senior Communities is now hiring a Community Clinical Liaison
Must be licensed RN or LPN
3 Evenings/week 5p - 8p and Weekends 9a - 6p (4-6 weekends off per year)
Remote position but must live in driving distance to Indianapolis, Indiana
The Community Clinical Liaison will work closely with our hospital partners and the trusted network of American Senior Communities throughout Indiana. Working with ASC nursing teams and hospital personnel, the Community Clinical Liaison will determine the patient's clinical needs and appropriateness for admission to skilled nursing care.
Requirements
- Current and valid LPN or RN license in the state of Indiana.
- Strong clinical assessment and customer service skills.
- Minimum of three years of clinical experience in an acute or long-term care setting.
- Demonstrates C.A.R.E. values to our residents, family members, customers and staff.
