Welcome House is looking for a compassionate Street Outreach Service Coordinator to connect unsheltered individuals and families with housing and essential services. This role involves direct outreach, collaboration with community partners, and case management to help break the cycle of homelessness.
Key Responsibilities:
- Engage with homeless individuals in the community, streets, and encampments.
- Connect them with emergency shelter, housing, and critical services.
- Provide transportation as needed.
- Work with outreach teams and community partners to improve services.
- Support team members and mentor staff on outreach procedures.
- Represent the agency in external meetings and maintain positive public relations.
- Maintain accurate participant records and update databases.
- Assist with data collection, analysis, and reporting for supervisors.
- Provide information for quarterly and annual reports.
