Hours: Monday - Friday, 8:00am - 4:30pm
General Summary
Supervises and oversees the work activities of Case Management social work staff and works in a team leadership model to develop and maintain departmental operations that are consistent with Hospital and System community needs. May carry a caseload and provides services to patients and families including counseling, discharge planning and referral to community services.
Essential Functions:
- Assists in selecting staff, trains/orients personnel, evaluates performance, prepares work schedules and makes recommendations for personnel actions.
- Assists in developing budgets.
- Identifies needs in program planning and facilitates program development. Supervises patient care, family communication, and record keeping.
- Collaborates with other hospital programs and community agencies with which patients and families are involved.
- Provides direct service to patients including interviewing patients and families, preparing social histories, identifying problems, and developing treatment.
- Prepares and maintains written documentation as required by the profession and the department.
- Provides therapeutic intervention with patient and family including individual, marital, parental, family and group therapies.
- Oversees the implementation and daily operations of the lean daily management system
- Assesses and ensures staff engagement through leadership rounding and employee engagement survey results
Common Expectations:
- Oversees/Conducts CM SW staffing and assignments, performance evaluations and evaluation and testing of competencies (as applicable). Provides orientation, training and continuing education of staff and initiates or makes recommendations for personnel actions.
- Demonstrates knowledge of the principles of growth and development over the life span of the assigned patient population. Assesses and interprets patient age specific data and provides appropriate, age specific treatment. Provides direct patient care to assigned patient age group(s).
- Demonstrates a commitment to patient, visitor and staff by: complying with all applicable safety regulations; learning the impact of medical errors and methodology that will lead to reduction of errors; reporting actual and potential errors, as well as hazardous conditions; identifying opportunities to standardize processes and "error proof" systems that will lead to increased safety; and participating in safety education programs and root cause.
- Maintains established policies and procedures, objectives, quality assessment, safety, environmental and infection control standards.
- Enhances professional growth and development through participation in educational programs, current literature, inservice meetings, and workshops.
- Provides outstanding service to all customers; fosters teamwork; and practices fiscal responsibility through improvement and innovation.
- Acts as liaison with nursing/medical staff and community agencies to facilitate problem solving and coordinate services. Acts as an educational resource.

Qualifications
Minimum Education:
- Bachelors Degree Required and
- Masters Degree Preferred
Work Experience:
- 2 years Relevant experience. Required
- Supervisory experience. Preferred
Licenses:
- Basic Life Support Upon Hire Required
- Licensed Social Worker Upon Hire Preferred
