Job Overview:
The ideal candidate will possess excellent patient assessment and care planning skills, with the ability to work independently or as part of a home healthcare team.
- Assess patient needs and develop individualized care plans to promote health, manage chronic conditions, and assist with recovery or palliative care.
- Administer prescribed medications, wound care, IV therapy, and other treatments as required.
- Educate patients and families on health conditions, medication management, self-care, and disease prevention strategies.
Key Qualifications:
- Education: Bachelor of Science in Nursing (BSN) preferred; Associate Degree in Nursing (ADN) accepted.
- Licensure:
- Valid Registered Nurse (RN) license in the state of practice.
- Certification:
- Basic Life Support (BLS) certification required.
- Home Health Nurse Certification (HHNC) preferred.
- Experience: Previous experience in home health care.
