About the Role

• Provide comprehensive patient care, including physical, emotional, and social assessments.
• Develop and implement individualized care plans that meet the unique needs of each patient.
• Collaborate with interdisciplinary teams to ensure continuity of care and improved health outcomes.
• Document patient care and progress accurately and thoroughly.

Requirements

• Valid Registered Nurse (RN) license in the state of practice.
• Basic Life Support (BLS) certification required.
• Home Health Nurse Certification (HHNC) preferred.
• Previous experience in home health care.
