They coordinate care across the continuum and engage with patients and families to ensure care needs are met. The role serves as a key resource for patients and a consultant to other care team members on psychosocial and resource needs. In conjunction with the patient and physician, they assess, coordinate, and implement a timely, safe discharge plan to the next appropriate level of care. They integrate and coordinate the patient's transitional care plan into their individualized discharge plans based on needs and available resources. Responsibilities include providing psychosocial assessments for patients to include timely and appropriate planning to advance the discharge plan. They assist in relaying information about community-based service offerings and offer guidance to assist with multi-system factors affecting psychosocial dynamics. They serve as a specialist on psychosocial and discharge needs, end-of-life care planning, and resource needs. They provide resource information necessary to aid patient / families in decision-making, including support for end-of-life care. They partner and offer feedback to the RN Case Manager concerning complex social determinants of health issues, situational dynamics, and social needs. Additional duties may be assigned. Core responsibilities include implementing discharge planning and providing resource information in a timely manner. They identify and document barriers for timely disposition. They understand eligibility processes for resources to assist in planning a safe transition for discharge. They respond to referrals for patient assistance from care team members. They participate in interdisciplinary rounds to confirm estimated discharge dates and make recommendations for care transitions. They initiate and facilitate post-acute referrals for timely transitions. They conduct financial needs assessments for patients requiring assistance for follow-up care. They advocate for and empower patients to make autonomous health care decisions, respecting cultural or religious beliefs. They develop protocols, procedures, and performance improvements to optimize patient outcomes. They complete initial and ongoing professional competency assessments and mandatory education. The Care Coordination Social Worker also serves as a preceptor and / or mentor for social work students if appropriate.
