```Job Overview```
We are seeking a skilled and compassionate Nurse Practitioner/Physician's Assistant to join our healthcare team. As a Wound Care Specialist, you will be responsible for providing specialized care and treatment to patients with acute or chronic wounds. Your expertise and attention to detail will play a crucial role in promoting healing and preventing complications.
```Duties```
- Perform comprehensive patient assessments to determine the type and severity of wounds
-Write wound care orders for our community resources
- Develop and implement individualized wound care plans based on assessment findings
- Cleanse and dress wounds using appropriate techniques and materials
- Monitor wound healing progress and make necessary adjustments to treatment plans
- Educate patients and their families on proper wound care techniques and prevention strategies
- Collaborate with interdisciplinary healthcare team members to ensure coordinated and effective patient care
- Maintain accurate and up-to-date patient records
-Order and interpret appropriate labs, radiological studies, and other tests per established criteria and monitor scheduling of patients for special tests
-Provide good bed side manner when in the patients home and the clinic.
```Skills```
To excel in this role, you should possess the following skills:
- Basic life support (BLS) certification
- Proficiency in wound assessment and management
- Strong case management skills to coordinate care across multiple disciplines
- Excellent patient assessment skills to identify potential complications or changes in condition
