Schedule - Monday through Friday Daylight hours (8:00 am - 4:00 pm)

$5,000 Hiring Bonus!

Job Purpose/Summary

The IDD LPN provides nursing services to consumers and nursing support to medical prescribers in the IDD Residential group home setting. The incumbent utilizes medical and behavioral health skills to assist with the removal of medical-related barriers to progress, as well as utilizes professional nursing processes to provide a holistic approach to nursing interventions. The position promotes mental health and physical well-being throughout treatment and crisis intervention. The IDD LPN utilizes technical nursing skills to administer, dispense, and safeguard medications stored at the various group homes. The incumbent performs nursing duties in accordance with standard agency practices and as per nursing license guidelines.

Minimum Qualifications

Education: Licensed Practical Nurse (LPN) licensed by the Commonwealth of Pennsylvania.

Experience:

  • 1-2 years of medical/surgical nursing experience and/or nursing home experience.
  • Experience working with psychiatric diagnoses or with adults with intellectual and developmental disabilities preferred.

Licenses, Certifications & Clearances:

  • Act 33/34 Clearances
  • FBI Clearance
  • Valid State of PA Class 1 Drivers License
  • Access to properly insured motor vehicle for client transportation and fieldwork and a safe driving record

Essential Duties & Responsibilities

  • Works with and assists all IDD leadership in promoting and maintaining consumers’ optimal health through observation, evaluation, and prompt medical intervention.
  • Travels to all IDD locations to assess and monitor consumers with extensive co-morbid medical conditions to facilitate preemptive and immediate intervention with any medical changes.
  • Prepares and administers medications as prescribed and as per agency policy and nursing license regulations. Assists in safeguarding medications stored on agency premises in accordance with agency policy.
  • Assists medical prescribers with ordering any diagnostic testing, as well as assessing needs for medication refills and any potential adverse reactions. Monitors consumers for reactions and side effects from medication administration.
  • Maintains clinical records. Completes documentation of all contacts with consumers and collaterals. Completes nursing review notes on a quarterly basis for all consumers. Updates records and forms, as needed, and reviews ECR/HRST for compliance with county and state regulations.
  • Reviews and follows up on daily health checks.
  • Develops and maintains a treatment plan, which reflects the consumer’s needs and goals. Reviews and updates treatment plan with multidisciplinary treatment as per department regulations.
