The Hospice Program Coordinator provides administrative support for the delivery of Hospice and Palliative services for patients, families, and volunteers. This role works closely with the Hospice Director in the day-to-day execution of non-clinical hospice services including patient record management, referral, intake, scheduling, and volunteer program support. Additionally, the Program Coordinator completes reports and maintains program compliance according to NHPCO and WSHPCO (state hospice organization) standards plus Medicare conditions of participation. The Hospice Program Coordinator is often the first point of contact with families and thus serves as a brand ambassador for Wesley. The coordinator should be well-versed in the hospice philosophy and should be comfortable speaking to families regarding Hospice and Palliative Care Services.
Salary Range: $21.60 to $31.77 per hour
