Position Summary
Under the direct supervision of Executive Director, Residential Administrator or designee, this position is responsible for developing and linking social service provider organizations with residents of a retirement community and their families, consulting with families, residents, and community management as needed. The Service Coordinator will assist in providing a dignified, respectful, and resident-centered environment.
Key Responsibilities
- Provides – Responsible for general case management, intake, and referral services to all residents in a senior living environment.
- Establishes – Creates linkages with agencies and service providers in the community and negotiates best service pricing.
- Builds a directory of providers for use by both staff and residents.
- Develops collaborative efforts for families, friends, volunteers, and community resources to create a support network for residents.
- Refers and links residents to service providers in the general community for case management, personal assistance, homemakers, meals-on-wheels, transportation, counseling, occasional visiting nurse, preventive health screening/wellness and legal advocacy.
- Develops – Constructs case plans in coordination with community assessment services.
- Educates – Keeps management team informed on issues related to aging in place and service coordination to expand their knowledge of the aging process and improve working with residents.
- Coaches – Trains community residents in the obligations of being a resident or coordinates such training.
- Educates the residents on service availability, application procedures, client rights.
- Assists – Helps residents build informal support networks with other residents, family, and friends.
- Maintains – Upholds current knowledge of federal and state regulations.
- Attends continuing education classes as required.
- Demonstrates continued professional work ethic and behavior.
- Performs – Additional responsibilities assigned as required.
Qualifications
Minimum Qualifications
- College degree in Social Work, Gerontology, Psychology or Counseling.
- Two or more years of experience in social services delivery required, preferably with senior citizens.
- Demonstrated knowledge of the aging process, elder services, disability services, and support services for the elderly.
- Basic knowledge of Medicare and supplemental insurance programs.
- Must be proficient in Microsoft office products.
- The ability to read, write and follow written/oral instructions in English.
- Excellent communications skills.
- Ability to multi-task and be organized.
- Supervisory or management experience preferred.
- Experience in a retirement community preferred.

Conditions of Employment
