What are the responsibilities and job description for the Social Worker position at WesleyLife?
As a Social Worker at Wesley on Grand, you will play a key role in supporting residents and families. In collaboration with our department directors, medical professionals, and community agencies, you will:
- Develop & Organize Programs: Plan, develop, and implement social service programs that align with resident-centered goals, ensuring that each resident receives the care they need to thrive.
- Provide Emotional support: Offer individualized emotional and psychological support to residents, helping them navigate the challenges of long-term care, and work closely with families for holistic care.
- Collaborate for Comprehensive Care: Partner with directors, medical professionals, and community agencies to address residents' psychosocial needs, ensuring a coordinated, team-oriented approach.
- Discharge Planning: Support smooth transitions for residents through effective discharge planning, ensuring all necessary resources and support systems are in place.
- Regulatory Expertise: Understand and guide residents and families through the Medicaid application process while staying informed on relevant Federal and State regulations related to long-term care.
- Care Conferences: Lead and contribute to care conferences, ensuring residents' goals are always at the forefront of planning and decision-making.
The pay range for this position is $26.00 - $30.70/hour. Starting pay rate will be based on years of experience.
