We are hiring for a Community Transitions Critical Time Intervention (CTI) Coach. This individual is responsible for providing community-based, time-limited, face-to-face interventions to individuals with the goal of supporting successful transitions and community integration. CTI is an intensive case management model designed to assist individuals with behavioral health challenges by linking them with clinical and community supports in an effort to decrease psychiatric hospitalizations and emergency department visits. CTI promotes a focus on recovery, and bridges the gap between institutional and independent living by helping individuals connect to services like mental health treatment, primary care, substance use, and housing over a 9-month period.
Locations: Claremont and Lebanon, New Hampshire
Responsibilities:
- Provide CTI interventions for an assigned group of clients including coordinating and monitoring the activities of a documented transition plan.
- Utilizing a person-centered approach, develop, implement, evaluate, and revise goals and plans in partnership with participants.
- Provide individual skill building to promote independent living and system management.
- Ensure immediate changes are made in transition plans as client’s needs change.
- Consult with community agencies and natural supports to ensure coordination in the service process.
- Conduct brief, evidence-based, and developmentally appropriate screening and conduct or arrange for more detailed assessments when indicated.
