We are seeking a Full Time Director of Social Services responsible for providing the services necessary to meet resident and family social and emotional needs and participating in the development of their comprehensive care plans.
Wage Information:
- $21.85/hour - $24.85/hour based on experience
- Including $0.85/hour employer paid supplemental pay until 9.30.25
Summary of duties:
- Assessing and completing the assigned sections of the (MDS) Minimum Data Set & Comprehensive Care Plan.
- Develop, Coordinate and Promote social interaction, reality orientation and intellectual stimulation.
- Conduct daily resident rounds
- Act as a liaison with social, health and community agencies.
- Assess, plan and document residents' discharge needs
- Coordinate family and community resources as needed to solve financial needs and promote emotional security.
Qualifications:
- Minimum one year of experience as social worker in a long term care environment or related field
- Strong Interpersonal and time management skills
