What are the responsibilities and job description for the Family Partner position at West Texas Centers?
The Family Partner is an experienced parent of a
child or children with serious emotional disturbance who provides peer
mentoring and support to the parents or primary caregiver of children in
services at WTC who are receiving a specific service package.
1. Will provide mentoring to parents of children in services with WTCMHMR
- a)Mentoring may include support, tips on parenting, and the ability to listen as well as provide some education.
2. Will work closely with the Case Manager
- a)Works closely with the Case manager to identify deficits in resources needed by the family or problems that the Case Manager may need to address with the family.
3. Will co-lead the Parent Support Group at the local
site with the Case Manager.
- a) Will assist Case Manager in leading the Parent Support Group at the local site.
4. Will become an advocate for the parent with WTCMHMR as
needed
- a) Includes open, regular communication with the Case Manager on a scheduled basis.
5. Will attend local CRCG meetings.
- a) Will attend local CRCG meetings if requested by the Case Manager on a case by case basis only.
6. Will complete progress notes
- a. Complete progress notes on each face to face or telephone visit with any consumer of WTCMHMR that h/she represents in this capacity.
- b. The progress note will be completed within 24 hours and given to the Case Manager.
7. Will keep a log of time spent with each parent,
mileage to and from the meeting place and topic covered.
- a. The Family Partner will keep a log of time spent with each parent, mileage to and from the meeting place and the topic covered.
- b. Log will be submitted to the Case Manager on the last day of each month.
- c. Case Manager will discuss the progress that the Family Partner has made in assisting the family(s).
- d. This log will be kept at the local site; however, a copy of it will be submitted monthly to the Director of MH Services.
8. Confidentiality
- a) The Family Partner will practice confidentiality with all information relating to the consumers he/she serves through this arrangement.
9. Reports possible abuse/neglect.
- a) Suspected or witnessed abuse/neglect will be reported immediately.
- b) Employee will report abuse and neglect as outlined in WTC policies and procedures.
10. Employee will comply with all WTC policies & procedures
related to Corporate Compliance involving Medicaid, Medicare and/or other pay
sources.
- a) Ensures service reporting documentation is accurate and correct. Documentation should reflect actual services delivered and for which reimbursement may be requested.
- b. Immediately report to the Center CEO any known or suspected fraudulent activity regarding delivery and billing of services.
11. Training
- a) Must take, pass and maintain all mandated training components.

- Property and Equipment Policies and related procedures
Staff will establish and maintain effective work relationships with individuals served and their families, supervisors, co-workers, and visitors by demonstrating cooperative, courteous, and respectful behavior. Staff will adhere to rules outlined in Confidentiality Statement signed by employee and follow Information Services policy/ procedure guidelines related to security and accountability as a user of WTC network and software application technology.
All team members will promote and ensures safety guidelines are followed for the work environment by maintaining clean, neat, organized and safe work areas. This includes but is not limited to the following:
- Safety concerns are reported to supervisor immediately upon discovery and/or corrected.
- Identifies and reports any unsafe factors in the environment.
- Reports and/or corrects safety hazards in the work area.
- Utilizes universal precautions when handling all blood and body fluids.
- Adheres to all standards, policies and procedures of infection control.
- Universal precautions are utilized in accordance to WTC policy and procedures.
Team Members must have and maintain a driving record that is acceptable for insurability through the Center's insurance carrier. Staff must maintain a safe, inspected and road-worthy vehicle and have a valid Texas Driver's License. Staff must notify supervisor immediately of any driving violations or citations received from law enforcement with no evidence of failure to report. Must meet the criteria that indicate a commitment to safe and responsible driving as defined by the Texas Risk Management Fund.
