JOB DESCRIPTION
POSITION : Transition Support Specialist
IMMEDIATE SUPERVISOR : Director and / or Coordinator of Special Education
FLSA / CONTRACT STATUS : Exempt
EMPLOYMENT TERM : Hourly
SALARY : $30 per hour up to 18 hours per week
EVALUATION : Performance in this position will be evaluated by the immediate supervisor and in accordance with
WV State Board Policy 5310 and Mineral County Board of Education Policy.
JOB SUMMARY : Provides support for special education students who are of transition age (typically ages 14-21) in
developing skills, knowledge, and resources necessary for a successful transition from school to
post-secondary education, employment, or independent living. The role involves working
collaboratively with students, families, educators, and community partners to create individualized
transition plans and ensure that students with disabilities achieve their post-secondary goals.
PERFORMANCE RESPONSIBILITIES :
- Collaborate with all stakeholders to provide information to eligible students;
- Attend IEP secondary meetings to provide parents and students with approved postsecondary materials, resources and referral information;
- Attend outreach activities as agreed upon by the Special Education Director and / or Coordinator of Special Education;
- Attend meetings with stakeholder groups to share information on Pre-Employment Transitions Services and activities;
- Provide training to middle school and high school students according to WVBOE Policy 2315 and Workforce Innovation Opportunities (WIOA)
- Maintain records and prepare reports and updates for program evaluation as requested by the Special Education Director and / or Coordinator of Special Education;
- Adheres to the Employee Code of Conduct as outlined in Mineral County School's Policy 3210; and
- Performs other duties as assigned.

Most commonly, the educational setting is indoors with outdoor settings for recess and field trip activities. The noise level in the work environment is low to moderate (20-60 dB). Exceptions include gymnasiums as well as vocational / technical laboratories where the noise levels may be moderate to loud (60-90 dB)
