Incumbent participates in steering committees, task forces, committee meetings, etc. to represent CSHCN, as assigned by supervisor. Incumbent maintains communication with all staff involved in the care of the client. Documents all contacts with clients/families and any other members of the medical home and ensures documents are forwarded to the state file as outlined in policy and procedure. Participates in professional and departmental/program training to maintain nursing license and improve job performance skills. Work with durable medical equipment and medical food vendors. Incumbent partners with families to assure they are informed about insurance coverage and benefit eligibility. Improve understanding of CSHCN by local community medical providers. Submit all required monthly reports to the State office as required by policy and supervisor.

Training: Baccalaureate degree in nursing from an accredited four-year college or university and two years of full-time or equivalent part-time paid experience as a registered professional nurse.
OR
Successful completion of an associate degree in nursing from an accredited college or university or a diploma nursing program and three years of experience as a registered professional nurse.
OR
Professional certification from a recognized national nursing certifying organization as a nurse or nurse practitioner in the area of assignment plus two years of full-time or equivalent part-time paid experience as a registered professional nurse.
OR
Successful completion of an associate degree in nursing from an accredited college or university or a diploma nursing program; plus baccalaureate degree in the health sciences or behavioral sciences and two years of full-time or equivalent part-time paid experience in nursing.
OR
Master's degree in nursing from an accredited four-year college or university.
Special Requirement: Current West Virginia license or temporary permit to practice as a registered professional nurse.
