What are the responsibilities and job description for the Family Coordinator position at Westbrook Health Services, Inc?
Summary: The Family Coordinator’s supports programs that promote emotional well-being, community-based prevention approaches, person-centered treatment interventions, and self directed or recovery support services. The Family Coordinator will coordinate a spectrum of community-based services that include partnerships with families and youth with the goal of improving youths’ functioning in their homes, schools, and communities.
Essential Duties and Responsibilities:
- Work collaboratively with other agencies and other divisions within Westbrook Health Services.
- Attend any local and state meetings as required by the grant and/or supervisor.
- Provide outreach to youth and their families.
- Provide supportive counseling services within the scope of training and competencies.
- Maintains confidentiality of all information and complies with HIPAA regulations.
- Comply with federal, state, and company policies, procedures, and regulations.
- Build connections between families, treatment teams, and community resources.
- Demonstrates knowledge of and supports Westbrook Health Services mission, vision, values, policies and procedures, operating instructions, confidentiality standards and code of ethical behavior.
- Completes trainings as required by the grant and/or Westbrook Health Services.
- Other duties as outlined by Westbrook Health Services.
This job requires travel in all 8 of Westbrook Health Service’s Counties. The majority of travel is during the work day, not overnight.
