Job Description
Job Title : Community Health Advocate
Reports To : Executive Director and Director of Operations
FLSA Status : Non-Exempt
Hours : 37.5 hours nonexempt staff position from 9am - 4 : 30pm. Some evening / weekend flexibility is required for community outreach and center special events.
Benefits : Health insurance, PTO, Holidays, 401K
Pay Range : $41,000 – $42,000
Summary : The Community Health Advocate is responsible for engaging and connecting neighbors in building a healthier community. This position works to coordinate services, fill gaps, make health connections, deliver education and engagement programming, make referrals, and serve as a case manager for our neighbors in need.
The Community Health Advocate will be a self-starter, compassionate, collaborative, organized, tech-savvy, well-informed individual able to communicate well in speaking and writing and advocate for the welfare of our neighbors.
Essential Duties and Responsibilities include the following :
- Case Management duties include
- Performing intake assessments, reviewing intakes done by other staff, and ensuring that identified needs are met
- Administering the Life Needs Assessment biannually and evaluating results
- developing individualized service / care plans in collaboration with program staff and track progress in Salesforce database
- Maintaining frequent contact with assigned individuals
- Utilizing HealtheConnections platform
- Utilizing the Unite Us platform
- Recruiting neighbors in need of services
- Assisting neighbors in obtaining other services as needed
- Leading classes or individual sessions of various kinds including health-based education, individual nutrition planning and community resource sessions
- Advance the initiatives of Healthy Alliance social care network and act as the primary staff person in advancing our work in the fee for service model.
- Attending relevant community meetings and events
- Work collaboratively with Westcott CC staff members to promote individual programs and Westcott CC as a whole
- Attend professional development as directed by the Executive and Director of Operations as well as on their own initiative
- Excellent verbal and writing skills required
- Excellent listening skills and the ability to assess program eligibility required
- Must be self-directed, able to prioritize activities, and manage time well
- Must be punctual to work and off-site meetings
- Must have strong problem-solving skills
- Must demonstrate a professional, welcoming, compassionate demeanor
- Ability to maintain reliable transportation
- Other duties as assigned by Executive Director and Director of Operations
Supervisory Responsibilities :
Supervisory responsibilities include instructing and training volunteers / interns in related work methods as well as assigning tasks and schedules.

This position requires regular use of computer, copier, and telephone as well as Microsoft Office, Salesforce, Unite Us and other software as needed, excel, database software.
Education / Experience :
Bachelor’s degree; or equivalent combination of education and experience.
Certificates and Licenses :
Salary : $41,000 - $42,000
