Job Description: As a Case Manager, you will play a pivotal role in supporting the emotional, social, and developmental needs of teens in our group home. You will work directly with youth to create individualized care plans, provide case management services, and coordinate resources to ensure each teen has the tools they need to succeed. This position requires strong communication skills, empathy, and the ability to work collaboratively with a multidisciplinary team.
Key Responsibilities:
- Monitor individualized case plans for each resident, focusing on their specific needs and goals.
- Advocate for the youth, ensuring they can access necessary services such as counseling, healthcare, education, and vocational opportunities.
- Facilitate regular case conferences with the youth, their families, social workers, and other professionals to review progress and adjust plans as needed.
- Provide crisis intervention and conflict resolution, maintaining a supportive and structured environment.
- Document and track all case notes, updates, and progress in compliance with agency guidelines.
- Offer emotional support and guidance to help teens navigate challenges and develop life skills.
- Coordinate transportation and supervise youth in community outings, appointments, and activities.
- Ensure compliance with licensing regulations, agency policies, and legal requirements.
Qualifications:
- Bachelor's degree in Social Work, Psychology, Sociology, or a related field.
- At least 1 year of experience working with youth in foster care, residential care, or social services preferred.
- Knowledge of trauma-informed care and experience working with adolescents, especially in foster care or group home settings.
- Strong communication and interpersonal skills, with the ability to build rapport with teens, families, and professionals.
- Ability to remain calm and composed in crisis situations, demonstrating strong problem-solving skills.
- Valid driver’s license and reliable transportation.
- Must pass background checks and drug screening.
