What are the responsibilities and job description for the Float Care Manager position at Western CT Area Agency on Aging?
General Statement of Duties:
This is full time position (35 hrs/wk) position that provides support to the Home Care Program Primary Care Managers who are temporarily away from the office on vacation or leave of absence. The Float Care Manager retains a modified client case load and responsibilities associated with case management of Home Care Program clients while assuming the absent Primary Care Manager’s case load. Inform the Primary Care Manager of any updates, concerns, or emergencies requiring follow up immediately upon Primary Care Manager’s return to work. Affectively implement or modify plans of care based on status changes in a timely manner to reduce client risk during Primary Care Manager’s absence. Correct any errors or inconsistencies with client chart that are identified and notify Primary Care Manager upon their return. Float Care manager will be responsible for completing monthlies or assessments/status reviews with Care Manager Supervisor. Check Primary Care Manager’s voicemails and return all calls within 24hours. Complete and file any required documentation in the Ascend system as it relates to the client’s needs during the Primary Care Manager’s absence. Required to adhere to all responsibilities as outlined in the Care Manager job description. Provides support to Care Managers working on complex client situations, completing monthly contacts or in any other valuable way during a time when a float case load has not been assigned.
Qualifications:
Candidates must be willing to work on site from the office during the initial 6 month period in between field work. Remote work is available to eligible employees after successful completion of this period.
Applicant must possess BA/BS with minimum 3 years Care Management Experience. Demonstrated familiarity with Connecticut Home Care Program for Elders policies and procedures and with community resources. Ability to establish and maintain positive, supportive working relationships with care managers. Demonstrate flexibility in assuming cases with little familiarity and filling in when needed to ensure clients’ needs are met, client remains safely in the community. Organizational skills, excellent time management, professional interpersonal skills, and knowledge of community resources. Assessing individuals in their homes, a nursing or other skilled facilities will be required as needed. Familiarity with computers is required as well as capacity to carry a laptop. Vehicle and or valid driver’s license required, travel throughout the 41 towns is required.
