We are looking for a Registered Nurse or LPN to be part of our Behavioral Health Home Program.
This person will be an integration specialist providing guidance on qualifying participants. Duties include meeting with clients to determine eligibility and make program recommendations, monitoring and overseeing client progress, provide guidance to team members.
Experience in a mental health setting preferred but not required. Must be able to work with an inter-disciplinary team and have excellent communication and documentation skills.
