The PACT (Program for Assertive Community Treatment) Nurse is a vital member of the healthcare team, responsible for providing comprehensive, patient-centered care within a primary care setting. This role involves coordinating and delivering care that promotes health, prevents disease, and manages chronic conditions. The PACT Nurse works closely with physicians, other nurses, and healthcare professionals to develop and implement individualized care plans, ensuring seamless transitions across various levels of care. Responsibilities include conducting patient assessments, administering medications, educating patients and their families on health maintenance, and facilitating communication between the patient and the interdisciplinary healthcare team.

Qualifications
A PACT Nurse will possess strong clinical skills, a compassionate approach, and a commitment to improving patient outcomes through collaborative, team-based care. They will have experience with medication education, prior authorizations, processing medication refill requests, care coordination, and other nursing tasks.
Minimum Qualifications:
- Bachelor’s or Associate’s degree in nursing, must be licensed as an RN/LPN
- One year of work experience in an inpatient or outpatient psychiatric setting preferred
- Ability to pass a background check upon offer of employment
- Valid current driver’s license with good driving record
