JOB DESCRIPTION
JOB TITLE: Assessment Coordinator
FLSA: Non-Exempt
DEPARTMENT: Homeless Services
REPORTS TO: Director, Homeless Services
LOCATION: White Plains, NY
SALARY: $60,000.00 - $75,000.00
SUMMARY:
Responsible for meeting, orienting, and assessing all new families placed in a Tier II emergency family shelter. Will maintain a small caseload until new families are assigned a housing-focused case manager. Provides supervisory guidance to all team members to help families quickly and successfully transition to permanent housing.
DUTIES & RESPONSIBILITIES:
- Conduct intakes with all new families entering the CFC (including initial ILP), complete coordinated entry,
- CHAT Assessments and other relevant family information on HMIS.
- Ensure that all families participate in assessment requirements (CASAC, medical, mental health).
- Maintain client demographic information and prepare statistical reports through MS Excel, HMIS, and
- other database systems.
- Plan and organize case management services in accordance with NYS OTDA and Westchester DSS
- regulations.
- Develop and oversee resident engagement activities including groups, workshops, training and
- recreational activities.
- Provide supervisory direction to case management team members regarding resident needs and
- interventions at the direction of program director.
- Liaise with Westchester DSS, schools, and other service providers to ensure continuum of care for families
- to ensure services are provided in a timely manner.
- Provide assistant, intervention, and follow-up to critical incidents with residents.
- Coordinate with the facilities, security, housekeeping, and childcare departments to ensure the physical
- plant operates safely, comfortably, and in compliance with state and county regulations.
- Support the Program Director with the daily operations and delivery of services to families in the shelter
- program.
- Must be available 24/7 unless on approved leave.
EXPERIENCE AND EDUCATION REQUIREMENTS:
MSW required (LMSW preferred). Six years of experience plus three years of supervisory and managerial experience working with children and families required. Strong assessment, direct practice, counseling, supervision, community outreach, and group leadership experience preferred. Ability to supervise social work interns (SIFI certified preferred). Excellent and demonstrated writing ability and communication skills. Advanced knowledge of MS Word, Excel, PowerPoint and database management skills. Experience in crisis intervention techniques and strength-based interventions. Fluency in Spanish is highly desirable.

Salary : $60,000 - $75,000
