We are seeking a self-driven and dedicated Certified Medical Assistant (CMA) to serve as the Patient Care Coordinator (PCC) to join our healthcare team. SUMMARY:
The Patient Care Coordinator will work cooperatively and effectively with the Director of Nursing, healthcare providers, and other team members to support the organization’s philosophy. The PCC works towards enhancing the quality of health care delivered and improving the satisfaction of patients served by West Caldwell Health Council, Inc.
Working within his/her scope of practice, the Patient Care Coordinator (PCC) will be part of the care management team and work in tandem with the internal healthcare team, external care management team, and community-based organizations, etc. to improve outcomes for high-risk patients. This role coordinates between health care services, recognizing the holistic needs of the patient.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
The Patient Care Coordinator (PCC) serves as a consultant to all disciplines and participates in all phases of the patient's care including direct and indirect patient care. This may include identifying community resources, making appointments, assisting with closing care gaps, assisting teammates and providers or referring to the internal Mental Health Provider as needed. They may also assist in coordinating a multidisciplinary team to meet the health care needs, including medical and/or psychosocial management of specified patients.
DAILY RESPONSIBILITIES:
- Works with the Medicaid (CMHN) Care Managers to schedule appointments, coordinate community referrals, communicate with patients and families about the medical home, and assist with health risk screens.
- Provides patient-specific information to be used in the daily huddle
- Alerts the providers to significant changes or abnormalities in the patient, and will provide information concerning their relevant condition, medical history and specialized treatment plans by other consulting practices.
- Participates in video teleconferences and other meetings as required
- Review hospital discharge instructions daily and transfer documents to the medical record
- Schedules appointments as needed
- Actively participates in teamwork by assisting others with the rooming process and providing direct patient care as needed
- Actively participates in triaging telephone calls
- Assists in completing prior authorizations as needed
- Shreds protected health information
- Facilitate clinical documentation distribution
- Complete other duties as assigned by the supervisor
