Job Requirements
High School diploma or Associates Degree in a Human Service field required, six (6) months to one-year related experience, or equivalent combination of education and experience. Incumbent should be able to apply common sense understanding to carry out instructions furnished in written and oral form, possess high-quality verbal and written communication ability, and possess strong interpersonal and organizational skills. Must be proficient in the operation of computer applications such as Microsoft Office, Publisher, fax, etc.
Job Summary
This position is primarily responsible for planning, organizing and implementing recreational, social, and therapeutic activities for the participants of social day program. Directly supervises and provides hands on, direct care to day program participants through the day for safety and social engagement.
Job Responsibilities
Assists with inspiring, planning and leading individual and group activities in the Social Day Program with persons who have dementia-related diagnoses.
Plans daily program prior to day of operation, based on structured calendar, assuring supplies are available for implementation. Notifies Program Director of supply needs in writing a minimum of one week prior to need.
Implements daily program of structured activities, leading all scheduled activities, for Social Day Program participants that is person-centered, engaging and therapeutically appropriate for persons with early and early middle stages of cognitive decline.
Assesses the environment for safety risks to program members, assuring member's safety throughout program operation. Evaluates and preserves program structure to impede wandering behaviors.
Directly supervises and/or accompanies members in person at all times during program operation. Meets all members at start of day and assures pick up at day completion.
Assists with transportation of residents from each program location, as needed.
Supervises lunch, sitting with members at dining table to assure member's ability to dine safely and independently without risk for choking or aspiration.
Leads set up and/or clean-up of area for scheduled activities.
Reports to Program Director all changes in member abilities or decline that may result in ineligibility or safety risk within program.
