What are the responsibilities and job description for the Resident Services Liaison position at Westminster-Canterbury on Chesapeake Bay?
DUTIES AND RESPONSIBILITIES
- Identifies psychosocial needs of residents and develops, promotes, and implements plans/programs to address and recognize needs, such as formal support groups. Interacts personally with residents and families.
- Assists in coordinating resident transfers to other levels of care including discussions with families and residents on the need for changing levels of care and works closely with the Move-In Coordinator on the logistics of the move.
- Assists with updates to the Enhanced Services future waiting list.
- Serves as a member of the Admissions Committee and conducts pre-admission interviews to determine if prospective residents meet established admission criteria.
- Assist new residents with adjustment and orientation to our community. Provides outreach to each new resident on moving day. Coordinates on-going New Resident Orientation to facilitate community orientation. Works closely with the Welcoming Committee to acclimate new residents to the community.
- Coordinates on-going WC Care & Services Monthly Education Sessions to facilitate community wide understanding of our continuum of care and services available.
- Coordinates with Director of Resident Life and Wellness and Chief Financial Officer to support residents who receive financial assistance from the W-C Foundation.
- Assists residents in making in-house transfers within the same level of care. Coordinates with Marketing and Chief Financial Officer to determine apartment availability and financial outcomes.
- Participates and provides input in Interdisciplinary Care Planning, Resident Review. Maintains necessary documentation consistent with regulatory requirements and policies for each level of care.
- Provides assistance and mediation to residents who are experiencing dispute and/or significant psychosocial problems.
- Screens Resident Hired Workers to ensure policy is being followed and safe-guards are in place to determine the qualifications of individuals to perform the desired work.
- Coordinates appropriate communication and updates to residents when needed to include Chatter articles, updates to Guidebook, Pet Directory, notices, and meetings.
- Assists Director of Resident Life with strategies and interventions to assure proper resident placement in the campus continuum and develops support systems for residents once a transition has been made.
- Assists with providing resources, education, referrals, and support to residents who are challenged by impairment such as decreased vision, hearing or mobility to empower them to be as independent as possible.
- Oversees volunteer programs for residents. Coordinates with the Executive Director of the Foundation to develop our annual Social Accountability Report.
- Serves on inter and intra departmental teams. Participates in Resident meetings and committees and represents Administration when requested. Provides facilitation when necessary.
- Assists with updates to electronic databases such as, but not limited to; Electronic Medical Records an Enquire.
- At the direction of the Director of Resident Life, assist The Hoy Center with Social Work discharge planning functions and recommending admissions based on referrals generated in Curaspan.
- Performs other duties as assigned.
WHAT YOU WILL NEED TO BE SUCCESSFUL:
- Bachelor’s degree in Social Work or a related field; Master's in Social Work is strongly preferred.
- Experience with Lifecare CCRC’s desirable. At least two years experience working with the aging population.
- Excellent communication skills and ability to mediate resident concerns
- Familiarity with Microsoft and Google applications.
- Demonstrate sensitivity and professionalism
- PHYSICAL REQUIREMENTS:
- Must be able to climb three flights of stairs.
- Must be able to lift 25 lbs.
