Social Worker
West View Nursing and Rehab is seeking a compassionate and experienced Social Worker to join our team. In this role, you will play a vital part in enhancing the quality of life for our Long-Term Care residents.
Responsibilities :
- Develop, implement, and evaluate social services programs for all Long-Term Care residents.
- Ensure adherence to all relevant State and Federal regulations.
- Advocate for resident rights and well-being.
- Collaborate with interdisciplinary teams (physicians, nurses, therapists, etc.) to provide comprehensive care.
- Conduct psychosocial assessments and develop individualized care plans.
- Provide counseling and support to residents and their families.
- Assist with discharge planning and community referrals.
